Due to the ever changing demands on the Surgery our registration policy changes dependant on the amount of patients we have registered at the time. Please ask one of the team on arrival at the Surgery.

Newhall Surgery is now

registering patients with a

MIDWAY, SWADLINCOTE or CHURCH GRESLEY address

If you know anyone who lives in the above areas who wishes to register with us please ask them to call into the Surgery to complete a New Patient Registration Form, or alternatively download a Registration Pack below, fill in the necessary forms for each person wishing to register and bring them into the Surgery. Each adult wishing to register at Newhall Surgery needs to provide two forms of identification – proof of identity and proof of address. These documents must include name, address and a photograph.

If you fit the registration criteria please collect a registration form from the Patient Services team and fill in the questionnaire. On completion you will be asked for proof of id/proof of address
