To register for our online services you will need to be patient here at the Surgery and complete this form. All patients wishing to use this service need to apply individually. Filling this form out does not automatically give you access, it will be at the discretion of the Surgery.

Upon receipt of the form it usually takes up to 7 working days for us to action your request. You will receive your username and password through the post.

Once registered you will be able to use the service to:

• Order your repeat prescriptions
• Make an appointment
• Cancel an appointment
• Change your contact details
• Review your medications and known allergies

A parent or guardian will need to be registered for the online services already in order for us to register a child as this will be linked to their account. When the child reaches 16 years of age, this access will be automatically removed.

If you have forgotten your password, please print and complete the ‘New Password Request’ form and return to the Surgery.
